Services are available to all graduates in good standing of Galen College, from any campus, past or present. To request these services, print out the necessary form below. Request that do not require a fee, such as Job Placement and course refresher requests, may be faxed. All requests requiring a payment must be mailed or dropped-off with payment. 

Normal turn-around time for document requests is 6-8 weeks and are processed in the order received. Any request that requires a student file to be retrieved

Any request requiring a student file to be retrieved from our offsite storage will be assessed a $20 fee.
